A treatment plan is a detailed plan that is tailored to the specific needs of each patient and is an effective tool for involving them in their therapy. Treatment plans usually have a straightforward format and include the following details:
Personal information, psychological history, and demographics of the patient
The current mental health problem's diagnosis
Treatment objectives with a high priority
Objectives that can be measured
A treatment timeline and a place to keep track of your progress
Usually last 45 minutes. During this session, the provider and client collectively and collaborative identify diverse intervention, discusses and agree on the best and favorable intervention.
